Following surgery and radiotherapy for head and neck cancer, persistent swelling (lymphoedema) and radiation fibrosis can cause significant, long-term complications. Dense scar tissue and fluid pooling around the face, jaw, and neck can restrict everyday movement, make swallowing difficult, and affect speech clarity.

Please note: Several linked case studies and clinical features include close-up clinical before-and-after photographs documenting post-surgical healing, scar softening, and swelling changes over time.

The Foundation of DEEP OSCILLATION®

The Foundation of DEEP OSCILLATION® (Video Presentation) A 9-minute overview

deep oscillation

DEEP OSCILLATION® is an athermic, non-invasive modality based on the Johnsen-Rahbek effect [7], utilising a low-intensity, pulsed electrostatic field established between the applicator (or vinyl-gloved hands) and the patient's skin. Rhythmic cycles of electrostatic attraction and release safely induce gentle, localised tissue microdeformations—microscopic changes in tissue shape and tension generated with minimal external mechanical pressure. Histological investigations have documented tissue-level changes consistent with anti-inflammatory effects, while clinical observations suggest potential benefits for local fluid dynamics and lymphatic drainage [1].

Because these microdeformations are induced electrostatically, the modality functions with minimal external mechanical load and requires no significant downward physical pressure. While these physical characteristics may make it a useful supportive option for delicate, hyper-sensitive, or post-radiotherapy cervical tissues, suitability must always depend on individual clinical assessment, tissue condition, and safety considerations.

Interacting directly with connective tissue, interstitial spaces, and lymphatic structures, diagnostic ultrasound observations have documented biophysical tissue responses at different depths (up to approximately 8 cm) under specific examination conditions [5, 6]. These observations contribute to our understanding of the physical interaction, but should be interpreted as reflecting biophysical responses under specific test conditions rather than establishing a universal therapeutic penetration depth. It is applied following individual clinical assessment with the aim of supporting tissue softening, improving tissue pliability, reducing perceived stiffness, and facilitating local fluid dynamics and tissue comfort.

Patient Testimonials and Lived Experience

The following accounts describe individual patient experiences. They illustrate patient perceived benefits but should not be interpreted as controlled evidence of treatment efficacy. In many cases, DEEP OSCILLATION® is used alongside self/simple lymphatic drainage (SLD), scar therapy, compression, taping, exercise, or other rehabilitation interventions.

Steve and Sandra Bradburn (Total Laryngectomy and Swallowing Recovery)

After total laryngectomy and neck dissection, severe cervical swelling constricted Steve's stoma, muffled his electrolarynx speech, and made swallowing solid food impossible. Following an initial NHS hospital trial, his sister Sandra crowdfunded a Personal unit for home use; twice-daily 25-minute sessions achieved rapid fluid clearance around his throat, enabling him to speak clearly and celebrate eating his first full normal meal since surgery.

Karen Liesching-Schroder (Mouth Cancer Foundation Support Group Lead)

A decade post-radiotherapy, Karen lived with severe nocturnal jaw spasms, "electric-shock" nerve twinges, and an impenetrable, rigid "woody" tongue. A 3-month home loan trial of Deep Oscillation delivered transformative relief: nightly self-treatment softened the hardened tongue tissue, eliminated nighttime cramps, and restored deep, uninterrupted sleep without reliance on pain medication.

Corinne (Living with Lymphoedema)

Living with chronic post-cancer head and neck lymphoedema, Corinne experienced heavy facial swelling, cervical tightness, muffled speech, and a complete loss of jawline definition. Serial photographic tracking documented immediate submental fluid clearance, visible tissue softening, and the restoration of facial symmetry from her very first session. Experiencing rapid structural and speech improvements gave her the confidence to invest in a Personal unit for daily home self-care:

"Lymphoedema is lifelong. The pain, suffering, and discomfort it brings do not have to be... Deep Oscillation treatment is painless and in fact actively relaxing, and to my astonishment I could see a visible difference to the swelling from the very first session... my jawline came back and my facial symmetry improved; my speech became clearer."

Amanda Winter (Oral Cancer Recovery, St Ann's Hospice)

Managed dense swelling beneath the chin following tongue reconstruction and neck dissection, alongside a tight forearm skin graft scar. Deep Oscillation helped soften both areas, improving comfort and facial symmetry.

before and after

Nick from Wales (Fibula Free Flap and Mandiblectomy Reconstruction)

Following extensive surgery for jaw cancer—including partial jawbone removal (mandiblectomy), neck dissection, chemotherapy, radiotherapy, and lower-leg fibula bone harvesting for microvascular facial reconstruction—Nick faced severe swelling and tight scarring across two distinct anatomical sites. Using the Deep Oscillation Personal unit at home over six months to treat both his neck and his lower leg donor site delivered noticeable dual-site decongestion, improved mobility, and dramatic scar remodelling:

"I started using the Deep Oscillation Personal supplied by PhysioPod UK just over six months ago, and since using a variety of programmes on my neck, head, and leg there has been a noticeable reduction in swelling. I have also been pleased with the healing of the scar on my neck, which is now barely visible, and the lower leg donor wound is also healing very well."

David McConnachie (Throat Cancer Survivor)

Two years post-treatment for Stage 2 throat cancer—following extensive surgery involving the removal of 150 lymph nodes alongside radiotherapy—David struggled with severe fluid pooling along his collarbone and debilitating neck stiffness that severely restricted head rotation. After nine clinic sessions with Deep Oscillation restored approximately 50% of his neck mobility, he invested in a Personal unit for daily self-care at home, allowing him to rapidly move trapped fluid out of his collarbone and regain everyday comfort:

"Deep Oscillation makes a profound difference to the quality of people's lives living with Head and Neck Lymphoedema... Using the unit allows me to move fluid out of the collarbone and neck quickly. It has given me a 100% improvement in comfort and daily function."

Mr A (67-Year-Old Survivor, Follow-up with Christine Talbot SRN)

Diagnosed with squamous cell carcinoma of the right tongue base, Mr A underwent chemotherapy, radiotherapy, and bilateral neck dissection. He developed debilitating late effects: severe rigid neck scarring, complete loss of chin-to-chest flexion and head rotation, frequent choking, excess salivation, and a hot, bulbous lymphatic mass behind his left ear. When standard manual lymphatic drainage reached a clinical plateau, specialist nurse Christine Talbot integrated Deep Oscillation. Over a 3-year longitudinal follow-up, regular minimal-pressure treatment resolved the bulbous ear mass, restored chin-to-chest flexion and cervical rotation, eliminated choking, and maintained zero cellulitis episodes across 18 months, giving him the confidence to rejoin his community through the local Men's Sheds:

"Deep Oscillation has improved my overall quality of life and has helped me manage the ongoing late side effects of head and neck cancer treatment and surgery. I would not have been able to cope with the ongoing effects of radiotherapy without Deep Oscillation treatment... Immediately after treatment I feel brilliant and get a very good quality of sleep that night."

Emergency Medicine Consultant and Mother of Two (Age 42)

Diagnosed with tongue squamous cell carcinoma (SCC) in 2020, undergoing radical surgery, neck dissection, chemotherapy, and radiotherapy. The multimodal treatment left her with severe facial and submental lymphoedema, dense tissue tightness, speech impairment, and restricted neck mobility that threatened her clinical career in acute hospital medicine. Applying Deep Oscillation produced immediate submental fluid clearance, softened rigid cervical tightness, cleared her speech articulation, and restored full head rotation:

"I had brutal treatment involving surgery followed by chemotherapy and radiotherapy... To my astonishment, I could see a visible difference in the swelling from the very first session. My husband noticed an immediate visual difference. My jawline returned, my facial symmetry improved, my speech became clearer, and my neck mobility returned, meaning I could sleep on my front and swim front crawl again. It gave me the confidence to return to my clinical NHS work. I only wish it was more widely available so that others could benefit as I have."

Chris Curtis (Founder and CEO, The Swallows Head and Neck Cancer Charity)

Living with persistent post-radiotherapy neck stiffness and restricted mobility, Chris experienced immediate, dramatic relief from his very first 15-minute session: "WOW what a difference! The pain had gone, and I had noticeably more movement. I would highly recommend speaking to an appropriately qualified healthcare professional about Deep Oscillation if you are recovering from surgery or radiotherapy."

Orna: Guided Daily Self-Care Model (Monica Conway MSc)

Following cancer surgery and radiotherapy, Orna lived with severe lymphatic compromise, persistent submental fluid pooling, and progressive cervical stiffness. Recognising that periodic clinic appointments alone could not prevent fluid reaccumulation and tissue re-hardening, her specialist therapist, Monica Conway, established a collaborative clinic-to-home protocol. After initiating clinical treatment with Vodder MLD and Deep Oscillation, Monica prescribed a Deep Oscillation Personal unit for daily home self-management. Performing short daily home sessions maintained tissue softness, continually cleared submental fluid, and prevented the return of fibrotic tightness between clinic visits—transforming Orna from a dependent patient into an empowered, confident self-manager.

Retired Survivor ("Regrets in Life? Just The One")

Diagnosed with neck cancer a decade ago—enduring radical surgery with 50+ staples, a feeding tube, chemotherapy, and radiotherapy—this 63-year-old retired survivor was left with severe cervical stiffness, a swollen chin, and persistent nerve damage down his neck and shoulder. Early hospital attempts with standard manual drainage struggled with his complex post-surgical anatomy. After specialist therapist Sue Oliver introduced Deep Oscillation in clinic, he purchased a Personal unit with a bespoke protocol tailored by Sue and PhysioPod. Sitting on the floor while his wife treats his neck and shoulders using vinyl gloves, daily home sessions dramatically expanded his cervical range of motion, decreased throat swelling, and improved nerve sensation across his shoulder:

"The treatment itself is completely painless and totally relaxing. I sit on the floor in front of my wife while she massages my neck and shoulders with vinyl gloves... If I stay awake for the full 30 minutes of the programme, it is certainly noteworthy! I don't have many regrets, but having experienced the machine for some years now, putting off the purchase of our personal unit until this year would be one of them."

In addition to individual case tracking, head and neck cancer survivors shared reflections in the PhysioPod® UK Phase 1 Independent Service Evaluation regarding the impact of Deep Oscillation on their daily recovery:

"The machine gave me back my quality of life... Deep Oscillation therapy made a significant reduction in swelling. I have experienced significant overall improvement." — Head and neck cancer survivor, UK Phase 1 Service Evaluation

"I was diagnosed with tongue cancer... Due to having the lymph nodes removed, I developed lymphoedema in my neck and cheek. I attended hospital where they used the deep oscillation machine on my neck and cheek; their treatment was fantastic and removed the fluid... The machine gave me back my quality of life, as it helped reduce the swelling in my tongue, which enhanced my speech." — Tongue cancer survivor (NHS hospital treatment & home user), UK Phase 1 Service Evaluation

"Deep Oscillation treatment is painless and in fact actively relaxing, and to my astonishment I could see a visible difference to the swelling from the very first session... my jawline came back and my facial symmetry improved; my speech became clearer." — Head and neck lymphoedema survivor, UK Phase 1 Service Evaluation

"Following treatment for a cancer on my jaw... I developed lymphoedema in the head and neck area, especially around the scar tissue at the operation site... I started using the 'Deep Oscillation Personal'... the swelling has decreased noticeably. I am pleased with the healing of the scar on my neck, which is now barely visible." — Jaw cancer survivor, UK Phase 1 Service Evaluation

"Deep oscillation allows me to take control... I use it daily. It is comforting and reduces the tight feeling in my neck." — Daily home self-care user, UK Phase 1 Service Evaluation

"I stumbled across Deep Oscillation by accident... it has been a revelation. I would say better results than MLD alone!" — Head and neck cancer survivor, UK Phase 1 Service Evaluation

Practitioner-reported clinical observations

The following accounts describe practitioner-reported observations. They illustrate perceived benefits and clinical experiences but should not be interpreted as controlled evidence of treatment efficacy. It is also important to note that in many cases DEEP OSCILLATION® is used alongside manual lymphatic drainage, scar therapy, kinesio taping, compression, exercise or other rehabilitation interventions.

Catherine Groom (Lymphoedema Services)

I have extensive experience in treating head and neck cancer patients who have developed secondary lymphoedema post-surgery and/or radiotherapy. This condition often presents with significant challenges, including excess swelling, fibrosis, pain, heaviness and tightness in the tissues, taut scars, and disruption in movement. Patients frequently experience difficulty swallowing and speaking due to swelling of the tongue and inside of the mouth, along with facial distortion. Beyond the physical symptoms, head and neck lymphoedema can have a tremendous psychological impact on a patient's well-being...

Since introducing Deep Oscillation we have found the results happen more quickly, especially when softening fibrosis or scar tissue... To be able to achieve effective results more quickly is hugely positive for the patient. It also frees up clinic slots to enable more patients to be treated."

Emma Holly (Founder, Restore Scar Therapy, Harley Street & UK Clinics)

Renowned UK and international educator on scar therapy, Emma Holly endorses Deep Oscillation as an "invaluable tool" for gentle, transformative oncology scar rehabilitation. She highlights how the pulsed electrostatic field releases deep fascial tethering, softens dense radiogenic contractures, and restores tissue glide across surgical incision lines and reconstructive donor sites (including forearm and fibula free flaps). Crucially, because it operates without downward physical pressure or friction, it enables effective scar remodelling even over fragile, irradiated skin and hypersensitive tissues where traditional manual scar massage is intolerable.

Monica Conway, MSc (Vodder MLD DLT Therapist, Ireland)

Clinical Insight: Details an integrated approach combining classical Vodder MLD with targeted Deep Oscillation protocols to soften dense post-radiotherapy fibrosis, release surgical scars, and mobilise trapped fluid. Monica champions a clinic-to-home model, guiding patients in daily home self-care (as with survivor Orna) to maintain tissue softness and prevent fluid rebound between clinic appointments.

Siobhán Casey (MLD DLT Practitioner, Oncology Scar Specialist & MLD Ireland Board)

Clinical Insight: Documenting a structured six-week clinical protocol (45 minutes weekly, Patient DK) in post-cancer secondary head and neck lymphoedema. Siobhàn observed marked, sustained oedema reduction across the face, periorbital area, and anterior/lateral neck, restoring anatomical soft tissue contours and reducing localised heaviness. Notably, she reported that Deep Oscillation significantly enhanced the redirection of lymph to alternative drainage territories compared to manual therapy alone—accelerating session outcomes while supporting swallowing ease, patient confidence, and social re-engagement:

"In entirety, the results demonstrate that Deep Oscillation is a highly effective, non-invasive, and patient-centred adjunctive treatment... delivering substantial and clinically meaningful improvements across all key domains—structural, functional, and psychosocial."

Lisa Capitelli, Ba (Hons) Dip RSM (The Body Mechanic Clinic, Aberdeen & Kirriemuir)

Clinical Insight: Documenting serial photographic case outcomes in private practice using the Personal Pro (50-minute full head and neck MLD routine with Deep Oscillation). Lisa highlights significant first-session fluid clearance, visible jawline restoration, and improved cervical flexibility. Importantly, she notes that the therapy produces a longer-lasting decongestive effect and overcomes the anxiety common in head and neck oncology:

"I have been very impressed with the changes I have seen in my clients who have had Lymphoedema in the head and neck, even after one session. Deep Oscillation actually makes the client relax due to the light buzzing noise it makes, which is very difficult with some patients. With doing a full head and neck MLD routine with Deep Oscillation, I find that the swelling reduces, flexibility increases and pain has minimised. I also find that the swelling does not come back as quickly or as severe as it was previously."

Lila Hipwell-Larken, BSc (The Treatment Clinic)

Clinical Insight: Details an advanced multimodal protocol combining Photobiomodulation (PBM), Fluoroscopy-Guided MLD (FG-MLD), and Deep Oscillation for Patient HMTTC, who presented several months post-surgery with severe bilateral neck swelling, an invisible jawline, mandibular pain, and severe lingual oedema that caused profound speech loss and a weight loss exceeding four stone. Lila highlights that Deep Oscillation made fluid relocation substantially easier while avoiding the painful downward manipulation the patient had feared. A single session achieved immediate tongue swelling reduction, restored jawline contours, and enabled instant speech return: "The machine gave me back my quality of life as it helped reduce the swelling in my tongue, which enhanced my speech." — Patient HMTTC

Regina Dengler, RGN (Specialist Lymphoedema Therapist, BLS, MLD UK Casley Smith)

Clinical Insight: Documenting the treatment of a 55-year-old female following transoral laser resection of a right tonsillar tumour, bilateral ("ear-to-ear") neck dissection, and six weeks of daily radiotherapy. Severe neck swelling, tissue pain, and persistent tongue oedema developed post-treatment, severely restricting tongue mobility and making eating normal food painful and difficult. Regina reports that following a single session combining MLD and Deep Oscillation, internal lingual swelling significantly decreased, restoring tongue range of motion and immediately easing the ability to chew and swallow:

"After the first treatment session her tongue swelling had reduced and she had more tongue movement, which in turn had made eating easier. She has improved further after regular, initially frequent, treatment sessions."

Sue Hansard (Retired Specialist Lymphoedema Nurse, First Lymph Care / St Ann's Hospice)

Clinical Insight: Reflects on years of hospice and oncology nursing, noting that Deep Oscillation provides a critical breakthrough for patients whose post-radiotherapy skin is too fragile or painful for firm manual touch. As the treating specialist in Amanda Winter's oral cancer recovery, Sue highlights how electrostatic application softens "woody" neck induration and speeds submental fluid clearance without mechanical pressure, while protecting therapists from physical strain.

In addition to named practitioner reports, healthcare professionals across the UK shared unedited reflections in the
PhysioPod® UK Phase 1 Independent Service Evaluation
regarding their clinical experiences managing Head and Neck Lymphoedema (HNL):

"With Deep Oscillation, you see quicker improvements in reduction of volume and fibrosis resolving versus stand-alone MLD. Thereby, patients require fewer treatments, resulting in shorter waiting times. Due to the effectiveness of Deep Oscillation, we are in the process of purchasing a unit for a home loan. This will decrease waiting times for those needing treatment." — NHS Lymphoedema Specialist

"Patients like it as you don’t need to apply much pressure on the neck, and you still get 8 cm down into the tissues. Wouldn’t be without my DOT now!" — Specialist Lymphoedema Practitioner

"Deep Oscillation provides reduction in firm tissue and fluid pockets, softening the tissue, particularly the midline submental area. Used alongside skin care, exercise and SLD, reduces clinic time if directing treatment." — Clinical Practitioner (Submental & Facial Fibrosis)

"Being able to treat HNL with Deep Oscillation therapy has been extremely significant in improving patient outcomes, allowing both visible and functional tangible benefits and improvements in quality of life." — Registered Lymphoedema Therapist

"The repeated proven effectiveness of Deep Oscillation's secret success lies in the depth of deliverance and the variable hertz settings per second. This unique scientific formulation delivers a stand-alone methodology unobtainable by a simple human touch technique... the improvement is instantly obvious to all in the room." — Oncology Rehabilitation Specialist

"Utilising Deep Oscillation therapy for head and neck lymphoedema is extremely effective — not only in the visual reduction of swelling, reduction of pain and improved mobility, but also in terms of speech and secretion management. Patients report accessing this treatment as life-changing." — Head & Neck Clinical Specialist

"Deep Oscillation has enhanced the treatment I can now give to my patients who suffer from Head and Neck oedema. It is a relaxing, painless treatment, which patients enjoy and feel the benefits of for several days. I would completely recommend this treatment for head and neck lymphoedema." — Lymphoedema Nurse Practitioner

"I have been very impressed with the changes I have seen in my clients who have had Lymphoedema in the neck and head, even after 1 session... By performing a full head and neck MLD routine, I find that swelling reduces, flexibility increases, and pain is minimised. I also find that the swelling does not come back as quickly and as severely as it was previously." — Private MLD DLT Practitioner

"DOT offers a number of benefits with a single device application without a number of adjunct manual therapies needing to be employed, resulting in more time per patient. Also, it acts deeper than is possible with manual therapy." — Specialist Practitioner

"The enhancement to MLD it provides is exceptional... The treatment has definitely improved quicker with Deep Oscillation than just MLD." — Senior Lymphoedema Therapist

"My patients report that this is the most impactful treatment offered. It has helped me cut down the time of appointments, therefore able to offer more appointments. LOVE IT!" — Lymphoedema Clinic Lead

"It allows less time for treatment, less workload and more time to focus on affected areas. I don’t get as tired as when doing MLD alone." — Lymphoedema Practitioner (Workforce Wellbeing Feedback)

Supportive Clinical Evidence

Journal of Clinical Medicine (Daia et al., 2026): "Deep Oscillation Therapy is an electrokinetic modality that combines the effects of pulsed electrostatic fields with mechanically induced tissue oscillations. Owing to its minimal mechanical loading and favourable safety profile... [it] may be considered a safe supportive intervention for selected oncology patients, particularly for the management of cancer-related lymphoedema (Oxford Level of Evidence 2)" [2].

In head and neck lymphoedema specifically, clinical investigations published by Halliday et al. (2020) in Springer's Applied Cancer Research evaluated incorporating DEEP OSCILLATION® as part of hospice-led multimodal rehabilitation protocols. The authors noted that low-frequency electrostatic impulses produce a tissue-relaxing effect particularly useful in areas harder to massage such as the head and neck, reporting measurable improvements on the Glasgow Benefit Inventory (GBI) Physical Benefit subscale (+13.1) [4].

Furthermore, the underlying biophysical principles and clinical parameters of the modality are formally documented in international medical literature, including Springer Nature's reference textbook Applied Lymphology [7].

Ultrasound & ICG Imaging

Foundational imaging studies were conducted on the extremities and illustrate general biophysical interactions across tissue layers rather than direct clinical equivalence in irradiated head and neck tissues. Accordingly, visual evidence of tissue motion demonstrates localised physical interaction, but should not be interpreted as establishing a universal therapeutic penetration depth or standalone treatment efficacy.

Dr. Solangel Hernández Tápanes, PhD, conducted pioneering diagnostic ultrasound investigations in Cuba to document the depth of tissue responses to DEEP OSCILLATION®, as a scientific prerequisite for clinical trials.
