ANNOTATION:
 

Introduction 

Currently, the number of patients with a new coronavirus infection continues to grow in the world. The COVID-19 virus disrupts the function of the entire oxygen transport system of the body, has a destructive effect on various organs and systems. Therefore, all those who have been ill, especially those who have had an infection in a moderate or severe form, need rehabilitation. Against the background of treatment, bronchial patency and drainage function of the lungs are restored faster, the psychoemotional state improves. 

Purpose of the study

To evaluate the effectiveness of the 2nd stage of medical rehabilitation in patients who have undergone a new coronavirus infection. Material and methods. There were 208 patients under observation, aged 32 to 88 years. Patients were admitted immediately after discharge from the hospital; the duration of treatment was from 10 to 14 days. A multidisciplinary team developed an individual program for each patient, determined goals and objectives. Rehabilitation measures included physiotherapy exercises, breathing exercises according to A.N. Strelnikova, inhalation of bronchodilators. In order to improve microcirculation and restore lung tissue, the treatment programs included: infrared laser radiation, magnetotherapy, electrophoresis. For greater accessibility of treatment in the conditions of salt caves, tabletop salt inhalers "Haloneba" were used. Manual massage was carried out in a gentle mode or deep oscillation on the device "HIVAMAT". With the help of an oxygen concentrator, some of the patients received low-flow oxygen therapy, the rest received an oxygen cocktail or lung lavage. All patients were examined by a psychologist for psychological correction. On admission and after the end of treatment, the patients underwent clinical and functional examinations: oxygen saturation assessment, hypoxia tolerance tests (Stange, Gench tests), mMRC dyspnea scale, Borg scale, 6-minute walk test, anxiety and depression intensity assessment (HADS) , assessment of the quality of life (EQ-5), etc.

Results 

Patients were admitted with complaints of shortness of breath, cough, weakness, sweating. The patients showed disorders of the psychoemotional background: subclinical or clinically severe anxiety in 16% of patients (9 and 7%, respectively), subclinical and clinically severe depression in 23% (14 and 9%, respectively). After a course of rehabilitation treatment, patients noted a decrease or disappearance of the symptoms of the disease. Was discharged with improvement in 97.0% of patients. Positive dynamics was manifested by an improvement in indicators for all assessment tests and scales. During treatment, the mean HADS anxiety score decreased from 4.09 ± 0.12 to 2.39 ± 0.36 (p <0.01), the mean depression score - from 4.27 ± 0.29 to 2.53 ± 0.44 (p <0.01). According to the assessment of the quality of life EQ-5, there was a decrease from 9.78 ± 1.45 to 8.14 ± 0.53 points (p <0.01). 

Conclusions

Thus, medical rehabilitation in patients with coronavirus infection ensures the normalization of respiratory function, helps to reduce anxiety-depressive disorders, and improves the quality of life.
