Patient Self Treatment Report: Use of Deep Oscillation Therapy for reduction of symptoms of endometriosis and ‘endo belly’

Introduction

 

Endometriosis is a disease in which tissue that normally grows inside the uterus grows outside it. The main symptoms are pelvic pain and infertility. Nearly half of those affected have chronic pelvic pain, while in 70 pain occurs during menstruation. Pain during sex is also common. Infertility occurs in up to half of women affected. Less common symptoms include urinary or bowel symptoms. About 25 of women have no symptoms. Endometriosis can have both social and psychological effects.

 

 

Background 

 

50 year old female with a 10 year history of endometriosis. Main symptoms were:

  • Painful heavy periods

  • Tar-like black blood during day 1 and often day 2 of the period

  • Dull, burning and stabbing pains a few days around ovulation

  • Pain and severe bloating in the week before the period

  • A notable increase over the last 6 months of ‘endo belly’ i.e. gradual increase in size of belly after ovulation as well as tenderness and a hardness. This distension still partly remained after each period had finished.

  • An uncomfortable tightness across the bottom of the abdomen just above the pubic region, like a tight elastic band cutting in, which was very noticeable during the week before a period. This sensation had appeared about 4 months previous and was getting gradually worse.

    For many years the patient followed an anti-inflammatory diet regime and took regular exercise to reduce endometriosis symptoms but was concerned about the increasing girth of her abdomen and the feeling of tenderness – like a bruised feeling, and tight band sensation. She felt that Deep Oscillation might be of benefit in reducing debris, inflammation and potential adhesions that had built up in her abdomen due to the internal bleeding that arises from endometriosis when the endometrial tissue bleeds into the abdominal cavity and the debris has no clear exit

 

Treatment Protocol

 

10 treatments of deep oscillation therapy on her abdomen, using the 9.5 cm oscillator head, moving it around the whole of the bottom of her abdomen and briefly on the upper abdomen.

  • Day1 & 3: 10 mins treatment @ 120Hz

  • Day 4 to 11: No treatment given as the patient was recovering from a leg operation and was taking anticoagulants while also on a menstrual period (from day 5).

  • Day 12 & 14: 20 mins treatment given @ 120 Hz

  • Days 15, 16, 17, 18, & 19 : Following advice, the regime was amended to 15 mins treatment @ 200Hz, followed by 5 mins treatment @ 40Hz

  • Day 20: 35 mins treatment @ 200Hz followed by 5 mins treatment @ 40Hz

 

Results

 

  • Within hours of each treatment the abdomen felt slightly softer and less tender.

  • After the first menstrual period on day 5 there was still some abdominal distension but this reduced gradually over the course of treatment.

  • Once the treatment finished on day 20 (day 16 of the patient’s menstrual cycle) there was a significant reduction of distension and tenderness of the lower abdomen.

  • During that menstrual cycle there was a noticeable absence of ‘endo belly’ which would normally appear and increase in the second half of the cycle. The abdomen was softer and flatter than it had been in months.

  • There was an absence of the tight band sensation in the week before the next menstrual period.

  • Significantly the next menstrual period (approx two weeks after treatment had stopped) was still heavy but not as heavy pre-treatment and notably there was a complete absence of black tar-like blood which had appeared at the start of a period pre-treatment.

  • The first menstrual period after treatment occurred on day 28 of the cycle which was longer than the previous few months during which the period had occurred on day 24, 25 or 26.

  • There was a noticeable effect of the treatment continuing to have an effect for many weeks after it had stopped. For the next 3 months there was a dramatic improvement with few endo-belly symptoms. The appearance of black blood appeared again very gradually after 3 months, and after 5 months the tight band sensation above the pubic region gradually started to come back.

 

Conclusion

 

The patient was very satisfied that the Hivamat Deep Oscillation Therapy had made a dramatic improvement in reducing symptoms of her endometriosis which included abdominal distension, uncomfortable abdominal tightness, tenderness, pain, and blood loss during periods. The length of the first menstrual cycle also extended to 28 days immediately after treatment (for that first cycle), which she felt was a good indicator. She felt as though her abdomen had been ‘cleaned out’ and reset again to near-normal, and that regular use of the Hivamat could potentially keep these symptoms to a minimum on an ongoing basis.
